I'm looking at a purchase of YouMania
Please can you confirm that the price of $38.00 is for the full version seen in the demo, including Virtuemart etc?

I'd also be looking to add the $15.00 Joomla Demo installation.

Could the extended forum support be added later if required?

Is anything else required other than a secure server?

Yes, price is for the full version as seen on the demo. Have a look here, this is exactly what you get: Demos included with single purchase.

In case of youmania, the demo installation comes indeed fully configured, this means all extensions, Virtuemart and K2. After the demo is installed you can then edit everything to your liking.

The extended forum support can be purchased anytime you like.

Last part of the question, no nothing else is required besides a proper host.
